On October 31, 2003, a Notice of Violation was issued for a Severity Level III violation involving the failure to secure from unauthorized removal or limit access to licensed material (two curies of molybdenum-99 in a molybdenum-99/technetium-99m generator, 20 millicuries of iodine-125 in 52 brachytherapy seeds, and 60 millicuries of cesium-137 in two sources) in a controlled area, and the failure to control and maintain constant surveillance of this licensed material.
